Form I-693, Report of Medical Examination and Vaccination Record, is required to determine whether an applicant for an immigration benefit in the United States is inadmissible under the health-related grounds of inadmissibility. For example, if a person has communicable diseases, severe mental health issues, and even substance abuse history, the person may fail to qualify and be denied immigration benefits. This examination must be completed for all immigration application submissions both in the United States and those submitted overseas and must be completed by an authorized USCIS civil surgeon, NOT a regular doctor. Failure to provide the appropriate Report, or for an unauthorized doctor to perform such exam may cause complications in an immigration case.
